Dementia Australia is part of the national ‘Aged Care Navigator Trial’ with a consortium of providers led by COTA and funded by the Australian Government. The trial involves testing different ways to share aged care information. Dementia Australia’s part in the trial is both an information hub as well as a dedicated Dementia Care Navigator accessible for greater WA. Our dedicated Dementia Care Navigator is here to assist anyone hoping to engage with, or learn more about the ‘My Aged Care’ system with the purpose of accessing government subsidised support in the home, or if needed, information about entry into Residential Care. For people in regional WA our Dementia Care Navigator can provide participants access to the trial by phone or email Participants can register their interest to be part of this FREE opportunity, by calling the National Dementia Helpline on 1800 100 500. If you know of someone living with dementia or alternatively seeking diagnosis, please ensure they are made aware of this opportunity to learn how to enter the My Aged Care System and use it to remain connected within their community. It is an opportunity to understand and begin the process of support guided by our Dementia Care Navigator.
